SERVICES
Keava Aesthetics provides non-surgical aesthetic treatments including:
* Botox®, Dysport®, Daxxify®
* Dermal fillers: Juvederm®, RHA®, Radiesse®, SkinVive®, Vamp™, Versa™
* VI Chemical Peels
* PRP and SkinPen® microneedling
* Vitamin B12 injections
All services are provided by a licensed Registered Nurse under the medical oversight of a Nurse Practitioner.
ELIGIBILITY
* Clients must be 18 years or older.
* Services are not covered by insurance and are self-pay only (cash, debit, credit card, or financing through Cherry.
* Aesthetic results vary by individual; outcomes are not guaranteed.
MEDICAL DISCLOSURE
* Clients are required to complete **medical history and consent forms** prior to treatment.
* Clients must disclose any relevant medical conditions, medications, or allergies.
* Keava Aesthetics reserves the right to refuse treatment if it is deemed unsafe or inappropriate.
APPOINTMENT POLICIES
* Cancellations/Reschedules: At least 24 hours’ notice is required.
* No-shows or late cancellations may result in a fee and/or denial of future bookings.
* Refunds: No refunds are given on services or products.
PHOTOS AND MEDIA
* Keava Aesthetics may take before, during, and after photos of treatments for medical documentation.
* These photos are the property of Keava Aesthetics.
* With client consent, photos may be used for educational, promotional, or advertising purposes (digital or print).
* Personal identifiers (such as name) will not be disclosed without explicit permission.
PRIVACY AND RECORDS
* All client information is kept confidential and handled in compliance with HIPAA regulations.
* Digital forms, records, and photos may be stored securely through Aesthetic Record.
LIABILITY
* By receiving treatment, clients agree to release Keava Aesthetics and its providers from liability for known and unknown risks associated with elective aesthetic procedures.
OREGON LAW
These Terms are governed by the laws of the State of Oregon. Any disputes will be handled under Oregon jurisdiction.
